Steering rack - How does inner rod "detach" itself???

Backed WC off of trailer and noticed steering rack rod disconnected. Which is fortunate for me, as I have been conducting WOT runs trying to get the clutches tuned the last several days. That would have been a nice wreck at 60 mph. This link (Log into Facebook) was probably the precursor as well, still can't find where the noise is coming from. Unclear how the inner tie rod can "unscrew" itself from rack. At any rate, what is the best way to rectify this? Can this be repaired? Replace it with the same factory steering rack? Is there a better option (aftermarket, etc) that is heavy duty?

Do threads feel ok? if so thread it in and run it. Maybe it was a factory miss and left loose...or just vibrated loose? There are or at least were a couple HD racks being made....use Google to find them.
I searched for a better rack and pinion setup and never found it. Other WC forums indicate that nothing like that exists. If you have info please share. The threads were buggered up. Ordered a new factory tie rod and sent steering rack away to be rebuilt as a preventive maintenance measure.
